Sepsis Alliance is preparing a new offering aimed at improving access to Sepsis Alliance Connect , the virtual support community for patients, caregivers, and loved ones. The project will include developing resources to share with patients who have been diagnosed with sepsis prior to discharge, to ease their transition from hospital to the outpatient setting. There are limited opportunities for acute care centers to provide feedback on these resources and be among the first to share them with patients.
